What Is Kratom?

Kratom (Mitragyna speciosa) is a tropical plant native to Southeast Asia. Its leaves contain dozens of active compounds called alkaloids — the two most significant being mitragynine and 7-hydroxymitragynine. In its natural leaf form, kratom has been used traditionally as a mild stimulant and pain reliever. In the United States, however, it is widely sold as capsules, powders, teas, and "shots" marketed as a natural alternative to opioid painkillers or as a way to self-treat withdrawal symptoms. Some of these products are many times more powerful than anything one could get from using the original plant. Saying these products are “all natural” is like saying heroin is “all natural” because its formula starting with a poppy.

Because kratom activates the same opioid receptors in the brain as drugs like morphine and oxycodone, regular use can lead to physical dependence and withdrawal symptoms that mirror those of traditional opioids — including muscle aches, insomnia, irritability, nausea, and intense cravings.

What Is 7-Hydroxymitragynine (7-OH)?

7-OH is the far more potent alkaloid found naturally in very small quantities — less than 2% — inside the kratom leaf. In recent years, manufacturers have begun producing concentrated, semi-synthetic 7-OH products: gummies, tablets, liquid shots, syrups, and even nasal sprays. These products deliver dramatically higher doses of 7-OH than the natural plant ever could, and they have flooded convenience stores, gas stations, and online retailers nationwide.

What makes 7-OH especially dangerous is its raw potency. Research shows that 7-hydroxymitragynine binds to opioid receptors with an affinity roughly 14 to 22 times greater than morphine. That means a single gummy or shot can deliver an opioid-like effect hundreds of times stronger than most users expect — and the risk of dependence, tolerance, and overdose rises accordingly. Clients have told us that even their first dose hit much stronger than they expected, and they developed tolerance and withdrawal effects within the first few doses.

Why this matters: Many people who start with kratom tea or capsules eventually move to concentrated 7-OH products for a stronger effect. If Kratom Has Been Used for Centuries, Why Is It Dangerous Now?

This is one of the most common questions we hear — and it's a fair one. In Southeast Asia, kratom leaves have been chewed or brewed into tea by farmers and laborers for hundreds of years, primarily as a mild stimulant to manage fatigue and physical pain. Historically, this traditional use did not produce the severe dependence, overdose, or respiratory depression we see today. There are several important reasons for that.

Fresh Leaves Contain Almost No 7-OH

Traditionally, users consumed fresh kratom leaves — either chewed directly from the tree or brewed immediately into tea. Fresh leaves contain virtually no detectable 7-hydroxymitragynine. The 7-OH compound is created through oxidation: it forms when mitragynine is exposed to air, heat, and light during the drying and processing that happens when leaves are prepared for export. Even in dried leaf material, 7-OH remains a trace compound at roughly 0.01–0.04% by weight. Modern concentrated products take this dramatically further — either extracting and concentrating naturally occurring 7-OH or producing it semi-synthetically to create doses hundreds of times higher than the natural plant could ever deliver.

Tea Has a Built-In "Ceiling Effect"

As Dr. Chris McCurdy, Professor of Medicinal Chemistry at the University of Florida, has explained in his research, the tea form of kratom is effectively self-limiting. Consuming too much natural kratom causes nausea and vomiting before a person can reach dangerously high opioid-like levels. This built-in mechanism acts as a natural safety brake that prevented traditional users from overdosing. Concentrated gummies, tablets, liquid shots, and nasal sprays bypass this protective mechanism entirely — delivering potent opioid effects without the body's warning signals.

Slower Absorption, Lower Potency

Brewing leaves into tea produces a preparation with slower, lower-bioavailability absorption compared to modern concentrated extracts. The body is exposed to far less active compound per dose, and it absorbs more gradually. This is fundamentally different from a concentrated 7-OH gummy or shot designed for rapid, high-potency delivery.

Respiratory Depression: The Critical Danger of 7-OH

Perhaps the most alarming difference is that concentrated 7-OH can suppress breathing. Research published in the British Journal of Pharmacology (Hill et al., 2022) demonstrated that 7-OH depresses respiration in animal models, and a 2025 study (Villalpando et al.) found that 7-OH was 4.5 times more potent than morphine at reducing minute volume — the total amount of air breathed per minute. The FDA's own toxicological assessment cites respiratory depression at more than 3× the potency of morphine as a primary concern. Traditional kratom tea at natural concentrations did not carry this risk.

The bottom line: The kratom plant that has been used in Southeast Asian communities for generations is not the same thing as the concentrated 7-OH products sold in American gas stations and online stores today. Modern processing has fundamentally changed the risk profile — and that is exactly why professional treatment is so important for anyone who has become dependent on these products.
