Consumer Rights, Medical Records, and Complaint Information

Windmill Wellness Ranch is committed to transparency, patient rights, and accountability. In accordance with Texas Health & Safety Code §181.105 and House Bill 4224, the information below explains how consumers may access their health care records, contact the applicable Council, and file a consumer complaint.

Requesting Your Health Care Records

Consumers may request copies of their health care records maintained by Windmill Wellness Ranch.

To request records:

  • Submit a written request or completed authorization form
  • Include the patient’s full name, date of birth, and dates of service
  • Specify the records being requested
  • Include a valid photo ID and authorization, if applicable
